The following forms have been developed by educators in the trenches.  Please feel free to use them as is or as a springboard for a form that works better for your school.

• The MYD Cheat Sheet (PDF) is a one-page overview developed by El Mirage Elementary School first grade teachers who wanted all the need-to-know information on one sheet, along with a place for taking notes about student behaviors.

• MYD Feedback - Common Area Expectations (PDF) (Littleton School District)  A form designed to maintain consistency in common areas throughout the school that uses a combination of a checklist and narrative information.

• Point Sheet - Daily (Prospect Middle School)  Prospect Middle School Staff developed a daily point sheet that describes all aspects of points.  This would be especially appropriate for students who would benefit from a more detailed description of their day.

• Point Sheet - Weekly (Prospect Middle School) An example of a weekly point sheet that gives parents more detailed information.

• Point Slip (PDF)  Heidi Bigelow, a Speech-Language Pathologist, at Park Place Middle designed a point slip that students can take back to their teacher.  This type of simple communication between the staff supports student responsibility and campus predictability.

• School-Home Communication Cover Letter - English/Spanish (West View School)  At times, it is necessary to mail the School-Home Communications because students are not showing them to their parents.  West View developed a cover letter to accompany those notes.  It is available in Word so that you may tailor it to your school.
